RESUMO

OBJECTIVE: This study explores how middle-aged Black Americans talk about race, without prompting, while telling their life stories. METHOD: Drawing upon a dataset of lengthy Life Story Interviews (N = 70), we first employed a keyword search to identify race-relevant interview scenes for each participant. Next, we conducted a thematic analysis of these scenes to identify salient racial narrative themes. Finally, we coded race-relevant scenes to examine the psychological correlates of racial narrative themes. RESULTS: We identified 460 total racially themed Life Story Interview scenes, with the number of racially themed scenes ranging from 1 to 17 across participants' interviews. Racial narrative themes included Community of Care, Black Cultural Identity, Multiculturalism, Activism, Encounter with Racism, Systemic Racism, and Racial Reckoning. Quantitative analyses highlight a relationship between racial narrative themes and psychological measures of wisdom and generativity. CONCLUSION: This study offers insight into the ways that race manifests in the life stories of Black Americans and highlights the importance of considering race in the study of narrative identity, and personality, more broadly.

Previous research has shown that young children deny being able to see an agent whose eyes are covered. The current study explored this phenomenon further. In Experiment 1, 3-year-olds denied that they could "see," but affirmed that they could "look at," a doll whose eyes were covered--indicating that they demand mutuality for seeing another but not for looking at another. In Experiment 2, 3.5-year-olds drew the same distinction between "see" and "look at" when facing a doll or a human. A strong correlation between children's knowledge of the reciprocal pronoun "each other" and their adherence to the mutuality demand was found. The results are discussed with respect to children's bias for second personal encounters and children's relational concept of persons.

RESUMO

Caso de paciente masculino 41 años, diabético tipo 1, trasplantado renal con tratamiento inmunosupresor, antecedente de carcinoma basocelular y espinocelular in situ tratado quirúrgicamente. Presentó en 2007 lesión nodular hiperpigmentada en pectoral izquierdo, se realizó exéresis con histopatología junio 2008: melanoma Clark III, Breslow 4 mm, ganglio centinela negativo. Clasificado como melanoma pectoral izquierdo Clark III Breslow 4 mm Estadio II (pT4aN0M0). Sintomatología gástrica noviembre 2009 estudios de imagen: hígado con lesiones nodulares múltiples tipo metastásicas, punción con aguja fina de hígado: melanoma metastásico. Reactivación a hígado múltiples metástasis, y metástasis pulmón derecho en diciembre 2009. Recibió tratamiento con temozolamida por ser paciente inmunosuprimido, con riñón único y baja depuración de creatinina, y limitación de quimioterapia con platino, recibió 3 ciclos, con progresión de enfermedad clínica e imaginológica, recibe segunda línea con interferón alfa-2b dosis modificadas, evolucionando tórpidamente, fallece por insuficiencia hepática complicada encefalopatía e insuficiencia renal crónica reagudizada.

Left pectoral classified as Clark III melanoma Breslow 4mm Stadium II (pT4aN0M0) May 2008 with multiple liver reactivation metastases, in November 2009 have gastric symptoms with imaging studies reported multiple nodular liver lesions type metastatic liver biopsy of metastatic melanoma and in right lung December 2009, was treated with temozolomide given immunocompromised patient treated with single kidney and low creatinine clearance with limited platinum-based chemotherapy received three cycles, showing progression of clinical disease and imaging, get second line with interferon alfa-2b dose modified, evolving torpidamente and died from liver failure complicated by encephalopathy and exacerbation of chronic renal failure.

Los síndromes linfoproliferativos posterior a trasplante renal incluyen un grupo de enfermedades linfoides que por definición se presentan después del mismo en un órgano sólido o en la médula ósea y están en relación con la terapia inmunosupresora administrada. La probabilidad de desarrollar una neoplasia maligna en un receptor de trasplante renal seguido a lo largo de 17 años, es de un 14 por ciento para cánceres no cutáneos, de un 47 por ciento en los carcinomas de piel y un 55 por ciento para cualquier tipo de cáncer. En este trabajo se describe un caso poco común de un paciente de 40 años con insuficiencia renal crónica secundario a hipertensión arterial en hemodiálisis, quien se le realizó trasplante renal de cadáver en el 2007, presentando alteración de función renal con masa palpable en fosa ilíaca derecha, en tratamiento con inmunosupresores. El examen histopatológico de la biopsia reportó: desorden linfoproliferativo posterior a trasplante con inmunohistoquímica positivo para CD20, CD79 en células linfoides B y CD45 en células linfoides T. Estudios de extensión metástasis en mediastino concluyéndose como: linfoma no Hodgkin inmunofenotipo B CD20+ estadio IV extra nodal renal con metástasis en mediastino superior mayo/2009 IPI score alto riesgo secundario a trasplante renal. Recibió tratamiento con R-CHOP obteniéndose respuesta oncológica completa, demostrado por CT-PET, actualmente libre de enfermedad, en controles nefrológicos. Se debe vigilar cualquier alteración clínica, ya que un rechazo de trasplante se puede confundir con una patología oncológica.


The posterior transplant lymph syndrome proliferative disorders include a group of renal lymphoid diseases by definition are present after a solid organ transplant or in bone marrow and are related to immunosuppressive therapy administered. The likelihood of developing a malignancy renal transplant recipient followed over the 17 years is 14 percent for the non-skin cancers, 47 percent in the carcinomas of the skin and 55 percent for any type of cancer. In this work we describes a rare case of a 40 year old patient with chronic renal failure secondary to arterial hypertension in hemodialysis, who underwent cadaveric renal transplant in 2007, the patient presented impaired renal function with palpable mass in right iliac fosse, in treatment with immune suppressants. The histopathology examination of the biopsy reported, posterior transplant with lymphoproliferative disorder with immunohistochemistry positive for CD20, CD79 on B lymphoid cells and CD45 on T lymphoid cells, the extension studies concluding that the mediastinal metastases were: Non-Hodgkin’s lymphoma immunophenotyping B extranodal CD20 + stage IV renal with metastases in the superior mediastinum IPI score May/2009 classified how secondary to high risk renal transplantation. The patient was treated with R-CHOP we obtained a oncology complete response, as demonstrated by CT-PET, and currently free of disease, in nephrology controls. It should monitor any clinical manifestation in the treatment of this kind of patients and served as a rejection of transplant can be confused with metastatic brain tumor.

RESUMO

El cáncer de mama representa en países en vías de desarrollo un problema de salud pública, en Venezuela en la última década su incidencia en la población ha ido en aumento. Su coincidencia con embarazo no es común y se asocia con niveles de ansiedad elevados tanto para el médico como para la paciente. La evolución y el tratamiento pueden tener consecuencias en el feto. Se presenta el caso de una paciente joven con cáncer localmente avanzado y embarazo cuyos estudios y tratamiento adecuado lograron un embarazo a término sin complicaciones.


The breast cancer represents in the development country a grave public health problem, in Venezuela in the last decade his incidence is increased. The coincidence with a pregnancy is not common, and is associated with high grade of anxiety to the patient and to the consultant doctor. The evaluation and treatment may be many consequences in the fetus. We presented and study a case of young woman with local advance breast right cancer and pregnancy, which studies and treatment was adequate and let her end of the pregnancy without a complications.

RESUMO

Los sitios habituales para metástasis de cáncer de próstata son en ganglios linfáticos regionales, vejiga, hueso, pulmón, hígado, un escaso porcentaje en otras localizaciones, como retroperitoneo y mediastínicas. Cirugía y radioterapia constituyen el tratamiento estándar de la enfermedad localizada. El uso de la terapia hormonal ha sido el principal abordaje oncológico en el tumor hormono sensible. La quimioterapia recientemente ha arrojado una alternativa terapéutica para estos tumores. Deprivación androgénica produce una disminución en niveles del antígeno prostático específico, regresión de masas tumorales medibles y período de estabilidad clínica. Paciente masculino de 60 años de edad, con adenocarcinoma poco diferenciado infiltrante de próstata Gleason 10, localmente avanzado, bajo tratamiento con bloqueo androgénico total, quien refirió dolor abdominal, aumento de volumen progresivo en hipogastrio y estreñimiento. Biopsia de tumor abdominal: adenocarcinoma de próstata poco diferenciado metastásico. Quimioterapia con docetaxel provee un mejor soporte para los pacientes con cáncer de próstata metastásico, andrógeno independiente.


The common sites for metastasis of prostate cancer are lymphatic’s regional nodule, bladder, bones, lung, liver, a small percentile of cases are present in other localizations how retro peritoneum, mediastinum. The surgery and radiation therapy are the standard treatment for disease localizes lesions. The use of hormonal therapy is the principal oncological treatment for hormone sensitive tumors. Recently chemotherapy treatment are and therapeutic alternative for this kind of tumors. Androgenic suppression produces a decrease of the levels for specifically prostate antigen, regression of the measure mass tumors anda clinical stable period. Masculine patient of 60 years old with less differentiated prostate adenocarcinoma Gleason 10, locally advanced, underwent suppression androgenic treatment, which refer abdominal pain and increase of the hipogastric volume. The biopsy of the abdominal tumor: Metastatic less differentiated adenocarcinoma of the prostate. Chemotherapy with docetaxel improves the support for the patients with metastatic prostate cancer metastásic androgenic independence.
